RECOVERY RESIDENCE ADMISSION REQUIREMENTS

 PROGRAM DESCRIPTION

The clients will live in a supportive alcohol/drug free environment in a community setting.  A Senior Resident in the Recovery Residence will monitor house activities.  An integral part of the Recovery Residence is community living, therefore the client's acceptance of responsibility for day to day living with other residents and participation in activities of the residence is seen as a major part of the treatment milieu.  The cost of the Recovery Residence includes the treatment program, housing and utilities.  Each client must provide their own clothing, bed linens for a twin mattress, blanket, pillow, towel, washcloth and personal toiletry items.  We also suggest some laundry detergent and a few dollars in change.  Food is not included in the program cost and each individual is responsible for providing for their own groceries.  However, we strongly encourage all residents to plan meals together and pool their resources in order to maximize their buying power.

The program requires a minimum commitment of nine months in Residence followed by nine months of Aftercare.

All clients must obtain a History and Physical upon admission.

The first two (2) week's program fees ($450.00) are due upon admission and are non-refundable and payment for the third week ($225) is due before the end of the first week of residency.  Thereafter program fees ($225) must be paid in advance on a weekly basis.  To reserve bed space in advance a non-refundable deposit of $450.00 is required and will be applied to the first two weeks of program fees.

The client is physically, mentally, emotionally and behaviorally capable of functioning in a residential setting and does not present a danger to himself or to others.

The client has abstained from alcohol and/or drug use at least twenty-four (24) hours prior to admission.

Clients must be medically stable.  Any individual with ongoing medical problems, including dual-diagnosis, may be accepted provided that they are addressing these problems with outside professional help, including medication management.  Primary Purpose Recovery Centers will assist with appropriate referrals when necessary, but it is solely the clients' responsibility to pay for such help.

All able clients are required to be employed or actively seeking employment.  Primary Purpose Recovery Centers will provide assistance in seeking employment, but it is the individual's responsibility to find and maintain suitable employment and to be able to meet the financial obligations necessary to remain in treatment.

Any client on disability and unable to work must be involved in appropriate activities during the day, such as educational or vocational training, partial day treatment or volunteer work.

The individual must be 18 years of age or older.

All cases of questionable admission must be cleared by the Program Director or designee.

 RECOVERY RESIDENCE RULES  

1.  NO DRINKING OR DRUG USE.  Any use of alcohol (including non-alcoholic beer) or illicit drugs is subject to immediate discharge from the program.  Use of any medication, prescription or over the counter, must be approved in advance by staff.  No products containing alcohol (cough medicine, mouthwash, etc.) are permitted.  Remember, you will be held responsible for any decision you make, so if in doubt, ask first! 

2.  Primary Purpose, Inc. is not responsible for any personal property lost or stolen nor any items left over 15 days by clients who have left the program, for any reason.

 3.  All residents are subject to random drug screens at no cost to the client.  Drug screens “for cause” (curfew violations, unusual behavior, previous positive result, etc) will be paid for by the client.  Refusal or failure to comply with a requested drug screen may be construed as a positive result and may result in discharge.

 4.  Visitors.  Any visitors to the residence must be approved by staff in advance.  NO FEMALE VISITORS ARE ALLOWED IN THE RESIDENCE AT ANY TIME OR UNDER ANY CIRCUMSTANCES!

 5.  No weapons, illegal activity, gambling, violence, abusive or threatening language or behavior, racial slurs, pornography/obscene materials or any adult oriented literature will be tolerated.

 6.  All residents must participate in maintaining the cleanliness and upkeep of the apartment..

 7.  SMOKING IS NOT PERMITTED inside the apartments or in the hallways of the building.  The only area approved for smoking is on the deck.  Violations of this policy may result in severe consequences, including discharge.

 8.  No VCR/DVD players, video game or stereo systems are permitted.  No TV’s are permitted in the bedrooms.  No resident may establish any service into the Residence without prior approval by staff (i.e. cable, telephone, etc.).  Computers may be allowed, at the discretion of staff, for work purposes only.  No access to the internet, games or DVD use will be permitted.

 9.  Curfew.  All residents are to be in the apartment by 11:00 PM.  No client is to leave the residence before 6:00 am.  Curfew times may be adjusted on an individual basis at the discretion of the staff.

 10. Passes.  Weekend passes are a privilege and may be applied for after completion of Phase I.  All passes are subject to approval by staff and are based on individual progress.  The pass is effective from 8:00 AM Saturday until 11:00 PM Sunday.  Passes must be submitted in person to treatment staff.

 11.  Honor system.  Residents are required to report to staff any resident who violates the rules of the residence.  Residents who violate the honor system may be subject to similar consequences, including discharge.

 12.  All residents are required to be out of bed by 8:00 AM Monday thru Friday and 10:00 AM on weekends.

 13.  All residents are required to attend at least one (1) 12-Step meeting every day they are not scheduled to be at the Center.

 14.  The telephone is to be used for talking with family, work, staff or in dealing with recovery oriented concerns only.  Calls are to be limited to 5 minutes.

 15.  All clients live in the Recovery Residence at the sole discretion of Primary Purpose and agree to vacate the property immediately if directed to do so by treatment staff.  All monies paid to Primary Purpose are fees for services rendered.
